When Dr. Zeeshan Kharashi, a neuro-radiologist, and his wife Dr. Aswa, a neurologist, looked at their biggest annual expense, they knew something had to change. Like many high-earning physicians, their tax bill was crushing their ability to build real wealth—despite their substantial incomes.

What happened next transformed not just their financial future, but their entire approach to creating passive income streams that could eventually free them from the demanding schedules of medical practice. Their journey offers a blueprint for any physician seeking financial freedom through real estate investing.

 

The Wake-Up Call: When High Income Doesn’t Equal Wealth

After eight years of practicing in Western New York, Dr. Zeeshan and Dr. Aswa were earning well but watching their hard-earned money disappear to taxes each year. “That was our biggest expense,” Dr. Zeeshan explains. “We were both really attuned to how much money we’re spending every year and looking at that figure.”

Their first real estate experience was accidental—they bought an expensive house in Princeton, New Jersey, planning to move there, then decided to stay put and rent it out instead. The result? They were barely breaking even, not even covering all their bills.

This common scenario led them to a crucial realization: “Everyone says real estate is a great investment vehicle. So it wasn’t working out that way for us. We went out to try to figure out, okay, what do other people know that we don’t?”

 

The Education That Changed Everything

The turning point came when Dr. Zeeshan attended a Passive Income MD conference, where he discovered the concept of real estate professional status (REPS)—a tax designation that allows qualifying investors to deduct real estate losses against their ordinary income.

For high-earning physicians, this revelation was game-changing. Here’s why REPS matters for doctors:

  • Massive tax savings: Real estate depreciation and expenses can offset medical income • Legal tax reduction: IRS-approved strategy specifically designed for active real estate investors
    Wealth acceleration: Money saved on taxes can be reinvested into more properties • Portfolio diversification: Reduces dependence on medical practice income alone

“After I came back and spoke with Aswa regarding REPS, that really changed everything for us,” Dr. Zeeshan recalls.

 

Making the Strategic Career Shift

Understanding the power of real estate investing for physicians, the couple made a bold decision. Dr. Aswa transitioned from full-time to part-time work on January 1st—a move that would allow her to qualify for REPS while maintaining her medical practice.

This strategic decision illustrates a key principle: you don’t have to choose between medicine and real estate investing. Many successful physician investors maintain their medical practices while building substantial real estate portfolios.

The timing proved fortuitous. When COVID-19 hit in March, Dr. Aswa’s reduced schedule and their real estate focus positioned them well to navigate the uncertainty.

 

Finding the Right Market Strategy

Like many new investors, the doctors initially struggled to find cash-flowing properties. They looked at Texas (where they had trained) and Phoenix, but couldn’t make the numbers work using proven investment criteria.

Their solution was a two-pronged approach:

  1. Local investing in Buffalo: Properties they could see and manage directly, perfect for achieving REPS requirements
  2. Appreciation plays in Tacoma: Balancing cash flow with long-term growth potential

This strategy addresses a common challenge for busy physicians: how to invest in real estate without it becoming another full-time job.

 

The First Deal: Going Big and Learning Fast

Rather than starting small, Dr. Zeeshan and Dr. Aswa chose to tackle a major BRRRR project (Buy, Rehab, Rent, Refinance, Repeat) as their first investment.

The Property: A duplex purchased for $95,000 that needed everything—roof, plumbing, electrical, and structural work.

The Investment: $97,000 in renovations (total investment: $192,000)

The Result: Property appraised at $250,000

The Outcome: They not only recovered their entire investment through refinancing but actually made money on the deal.

Key Success Factors

Strategic Partnership: Dr. Aswa’s analytical eye caught issues Dr. Zeeshan might have missed. “She was like, ‘well, look, the roof is awful. Like, you know, you need to calculate that into your returns,'” he explains.

Network Building: The project connected them with reliable contractors, HVAC specialists, and other professionals—creating a team for future deals.

Learning Through Action: “We’re not afraid of problems that crop up now,” says Dr. Aswa. “Anything now is very much tackled because we’ve seen probably the worst of the worst.”

 

The Timeline and Challenges

The project timeline stretched from summer to the following spring due to: • Market delays in closing • COVID-19 construction slowdowns
• Contractor challenges (including their HVAC contractor being hospitalized with COVID)

Despite setbacks, their persistence paid off. The 8-9 month timeline from purchase to refinance delivered exceptional returns and invaluable experience.

 

Tax Benefits in Action

By achieving real estate professional status and actively managing their properties, the doctors unlocked significant tax advantages:

  • Depreciation deductions against ordinary medical income • Expense write-offs for property management, repairs, and improvements • Professional development costs for real estate education and networking • Travel deductions for property visits and market research

These benefits can save high-earning physicians tens of thousands of dollars annually in taxes.

“Don’t overthink things, just start. That was the hardest part, just getting started for us,” advises Dr. Zeeshan. “The best time to buy real estate is right now.”
